When Is ABO Genotyping Useful? 

ABO genotyping is particularly valuable in situations where routine serology does not provide a clear or reliable result, including: 

  • ABO typing discrepancies, such as front and back type discordance or weak or missing antigen expression 
  • Weak or variant ABO phenotypes, such as suspected subgroups of A or B, or unexplained mixed-field reactivity
  • Patients with recent transfusion or transplant history, which can complicate routine serologic typing
  • Neonatal and prenatal testing, which can be complicated by low sample volume or passive transfer of antibodies
  • Patients with hematologic malignancy, in which antigen expression may be altered due to disease or therapy  
  • Identification of A2 solid-organ donors, when serologic testing is inconclusive
